STOP Documents

While we highly recommend that all sites use the STOP Portal as much as possible, we recognize there may be instances where paper forms are necessary. Remember, consent forms should always be completed using a paper copy.

Key point to remember if using paper forms:

• All enrollment (registration and baseline) and visit forms need to be entered into the STOP Portal before being sent to STOP.
• Do not discard source documents if they contain participant information.

STOP will make arrangements on a monthly basis to collect all paper documents. For privacy reasons, consent forms (which contain participants’ names) are sent in separate packages from baseline, registration, and visit forms.
